NWI Coyotes Mini Camp Permission & Liability Waiver

I, the undersigned parent/guardian, give permission for my child, to participate in the NWI Coyotes Mini Camp. I understand that participation in sports activities carries inherent risks, including the possibility of injury or illness. By signing this waiver, I release and hold harmless NWI Coyotes, its coaches, staff, and affiliates from any and all liability for injuries, illness, or damages that may occur during camp activities. I acknowledge that I am responsible for any medical expenses incurred as a result of my child’s participation. I confirm that my child is physically able to participate and agree to assume full responsibility for any risks associated with camp activities.
