Registration: $175

Jersey Fee: $60

Registration fees and Jersey Fees are due at fitting day. Payment can be made at the bottom of this form with CC/Apple Pay/Google Pay/ Paypal, send a check to NWI COYOTES at 5038 E Rock Grove Road, or bring cash/check payment to fitting day.

NWI Coyotes provides all helmets/pads/and practice jersey & pants, mouthguards. Athletes are responsible to provide socks, cleats & water bottles.

All players from surrounding communities and school districts are welcome to join. Please be advised that 6 year old players may receive limited game time as they are learning basic fundamentals of the sports.

Returning Players: You have the "rights" to your jersey number. Prior year jerseys can be reused if they still fit properly and are in good shape. *A personalized game jersey is required for all players*

Practices: Start the last week of July and run through the last game in October. Practices are Tuesday through Thursday from 5:30 pm to 7:30 pm at the Jane Adams Community Center in Cedarville IL.
YOUR CHILDS ATTENDANCE AT PRACTICE WILL INFLUENCE GAME TIME.

Games: Season is typically 9 weeks with a one week bye (8 games) All games are primarily on Saturdays, but may be moved to Sundays if there is a shortage of referees, beginning August 12th.

ALL participants will need to send a copy of their birth certificate to finalize registration.

NWI Coyotes is a volunteer ran program. Volunteering is a requirement to be a part of the program. All families will be expected to volunteer at a MINIMUM of 2 home games.
Payment can be made here via card or brought to any parent meeting or fitting day.
